Clinical Observation Module #004: The Case of the Choosing Intern
This time, Dr. Okafor doesn’t just train the husband. She tests what he becomes.
A Westwood Wellness Interactive Assessment — Part One of Two
Module #001 introduced you to Westwood’s internship program. You reviewed a case file. You learned what happens when a man’s measurements fall short of his wife’s requirements.
Module #002 put you in the room. You watched Dr. Hailey measure Mr. K. You watched his wife learn why she couldn’t sleep. You watched a diagnosis become unavoidable.
Module #003 gave you an entire week. You observed Ryan Harrison’s training — from intake to oral proficiency to permitted release. You met Dr. Okafor. You met Addison. You wrote your clinical report.
Now Okafor has a new case. A Hailey referral — and when Hailey walks the file over herself rather than sending the notes, you know it’s serious.
A married couple. Twelve years together. A wife who called the clinic herself and said cock in the first ten minutes.
And a husband whose penis confirmed everything before his wife finished her first sentence.
What’s Different This Time
Everything.
Module #003 followed one man through a week of training. You observed. You assessed. You wrote your report.
Module #004 puts you in a couples assessment — both partners in the room, both monitored, both responding. You will watch a wife’s body discover what adequate penetration feels like while her husband stands restrained at the foot of the bed. You will watch Dr. Okafor prevent his ejaculation with her hand on his testicles and her voice in his ear.
And then something no previous module has attempted: the assessment doesn’t end with a diagnosis. It ends with a question.
Three possible configurations. Three different futures for the Perkins marriage. Okafor needs your recommendation.
You choose.
Your answer determines how this story ends — and what Part Two contains.
Your Role
This is Part One of a two-part module. When both parts are completed, I anticipate it will be the longest single Typeform story I’ve written — significantly longer than Module #003 — but, each part alone should be more manageable. It’s why I split it in two. Still, it will take time. Plan accordingly.
You will begin in Dr. Okafor’s office. Bergamot and old paper. She will brief you on Billy and Mara Perkins — his dimensions, his arousal architecture, her nine years of fantasy dependency.
You will follow Addison to the waiting room and meet the couple. You will watch Addison say your penis to Billy twice before he leaves reception.
You will enter the assessment suite — a new space, purpose-built for couples, with mirrors at every angle and cameras recording everything. You will watch Okafor test six phallic surrogates, isolating girth and length independently, while Billy watches from a floor marker and Mara’s body tells the truth her mouth spent twelve years hiding.
You will watch Mara orgasm on a surrogate nearly twice her husband’s size. You will watch Billy nearly ejaculate — and you will watch Okafor’s hand find his testicles and hold him there, preventing it, because she is not finished with him.
Then the assessment shifts. Okafor explains three possible configurations for Billy’s role in a supplemented marriage:
The Three-Position Protocol. The Sister-Wife Configuration. Participatory sharing.
She cannot test with questions. Men lie about their arousal — not deliberately, but structurally. So she tests with stimuli. Lingerie. A mirror. Six cue cards read in his wife’s voice, escalating from you look so pretty to his cock in your pussy, then in my mouth.
His penis responds to each one. The monitors capture everything.
And then Okafor turns to you. She wants your clinical recommendation. Which configuration fits Billy Perkins?
You decide. The story branches. Your ending is yours.
Part Two — Billy’s training, tailored to your recommendation — arrives once all the data is in.
A Note on Dr. Okafor
Those of you who completed Module #003 have met her. For those who haven’t: Dr. Adaeze Okafor is the Director of Westwood’s Responsive Male Training Program. Late sixties. British-Nigerian. Close-cropped gray hair and eyes that miss nothing and forgive most of it. She is Hailey’s mentor — the woman who was doing this work before Hailey gave it a name.
Where Hailey diagnoses and Moreau comforts, Okafor trains. She is the practitioner. Her gift to the responsive male is not a framework or permission. It is competency — she makes him useful.
In this module, you will see a side of her the Harrison case only hinted at. She is warmer than you expect. She is more direct than you are prepared for. And when she holds Billy Perkins’s testicles in her hand and tells his little man to listen, you will understand why Hailey walked this file over personally.

A Note on Clinical Files
If you are a paid subscriber and wish to have your responses recorded to your personal Westwood clinical file, you may enter your email at the end of the module. There is absolutely no obligation to do so.
Your responses remain confidential. But over time, as you complete additional assessments, your file builds a comprehensive profile of your arousal patterns, recognition responses, and progression through observation.
Free subscribers may complete the module but clinical file registration is reserved for those enrolled in Standard Care or Intensive Personal Care.
If you’re not yet a paid subscriber and today’s session surfaces something worth exploring further, you know where to find us.
Dr. Okafor will see you now.
