Clinical Observation Module #005: The Case of the Sissy and his Wife
Dr. Okafor doesn’t just train the husband this time. She names what he is — and his wife names who he becomes.
A Westwood Wellness Interactive Assessment — Part Two of Three of the Perkins Case
Module #001 introduced you to Westwood’s internship program. You reviewed a case file. You learned what happens when a man’s measurements fall short of his wife’s requirements.
Module #002 put you in the room. You watched Dr. Hailey measure Mr. K. You watched his wife learn why she couldn’t sleep. You watched a diagnosis become unavoidable.
Module #003 gave you an entire week. You observed Ryan Harrison’s training — from intake to oral proficiency to permitted release. You met Dr. Okafor. You met Addison. You wrote your clinical report.
Module #004 put you in a couples assessment. You watched Billy Perkins’s wife orgasm on a surrogate nearly twice his size while he stood restrained at the foot of the bed. You watched Dr. Okafor test three possible configurations — lingerie, a mirror, six cue cards read in his wife’s voice. You filed your recommendation.
Now Dr. Hailey has reviewed the data. She knows what Billy’s body is reaching for. She also knows he isn’t ready for it yet — and neither is Mara.
Before they can share, they need a girlfriend.
What’s Different This Time
Module #004 ended with a question: which configuration fits Billy Perkins? You answered. Your data was recorded.
Module #005 begins with Dr. Hailey’s answer — and it isn’t the one most of you gave.
Your recommendations favored Participatory Sharing. Hailey favors patience. She sees what the peak arousal data showed, but she also sees what it didn’t: that Mara needs the relational repair before she can share the adequate male, and that Billy’s body must arrive at sharing through the configuration that precedes it.
The Sister-Wife comes first.
In this module, you will watch Dr. Okafor teach Billy things no one has ever taught him — how a girl grooms, how a girl dresses, how the clothes hold a charge that his body cannot resist. You will choose his outfit. You will watch Addison manage his erection with clinical efficiency. You will watch the refractory data confirm what Okafor suspected: the clothing overrides everything.
Then Billy returns with Mara. And everything changes.
You will watch Mara dress her husband for the first time. You will watch Okafor use a word that no one in this clinic has spoken aloud before — a word Billy’s penis has been saying for three weeks. You will watch Mara look at the person in the mirror and struggle to describe what she sees. You will watch her say “good boy” — and then, at the moment it matters most, “good girl.”
You will watch a wife give her husband a name.
And you will watch two girlfriends wash panties together in a basin while the monitors record something Dr. Okafor has been waiting to see: convergent data. Both of them responding. Both of them arriving.
Your Role
This is Part Two of the Perkins case — the Sister-Wife Configuration. It picks up where Module #004 left off and delivers the first phase of Billy’s progressive training.
You will begin in Dr. Hailey’s office, where she will walk Billy and Mara through the assessment data and explain the protocol. Then you will follow them to Dr. Okafor’s training suite, where the work begins.
You will be asked to make choices — what Billy should wear, how he should groom, whether you like the name his wife gives him. You will be asked to rate your arousal at key moments. And at the end, you will be asked the question that matters: is she ready to share?
Part Three — the escalation to Participatory Sharing, the adequate male, the final tableau — arrives once Okafor has reviewed the data from today’s session.
A Note on Language
This module introduces a word that Dr. Okafor uses clinically and without judgment. The word is sissy. She defines it precisely: a feminized man. Not a woman. Not transgender. Not gay. A man who is feminized, who occupies a feminine position in his marriage, who responds to feminization with arousal and recognition. A man who is both husband and girlfriend — and the double image is the finished picture.
If that word means something to you — or if you’ve been waiting for someone to say it without making it a slur — this module may be for you.
If you’re not sure how you feel about it, this module may also be for you. Okafor will explain. Your response is data.
If you’re not inclined toward feminization or the blurred edge of what masculinity becomes, this module may not be for you.

A Note on Clinical Files
If you are a paid subscriber and wish to have your responses recorded to your personal Westwood clinical file, you may enter your email at the end of the module. There is absolutely no obligation to do so.
Your responses remain confidential. But over time, as you complete additional assessments, your file builds a comprehensive profile of your arousal patterns, recognition responses, and progression through observation.
Free subscribers may complete the module but clinical file registration is reserved for those enrolled in Standard Care or Intensive Personal Care.
If you’re not yet a paid subscriber and today’s session surfaces something worth exploring further, you know where to find us.
Dr. Hailey will see you now.
